Delivering Personalised Support for Older People living in Care Homes in Flintshire

We want to improve the day to day experiences of people living in Flintshire care homes by enabling them to make more choices about the things that matter most to them and by knowing how best to support them.  Progress for Providers sets out clearly our expectations about the delivery of individualised support by residential care providers and supports managers and staff teams to promote people to have more choice and control in their lives.

 We have developed 5 key statements to describe what we are doing and why we are doing it. 

  1. Really knowing what matters to people and what great support looks like is key in achieving quality of life
  2. It’s not enough to know what matters to people, we have to use this information to make an everyday difference to their lives
  3. Knowing what matters to staff will highlight connections and shared interests and make the working day more enjoyable
  4. Families and friends know their loved ones best and can help us to get it right
  5. Progress for Providers will help check where we are and what needs to be done next
