Putting you at the centre of your care

Each of us has our own likes, dislikes, hobbies and interests. We are all different, and that’s what makes life interesting. In Flintshire, we recognise everyone for their individuality and in social care, we aim to do as much as possible so you can remain independent, have choices and enjoy the things you like to do. We call this ‘person-centred care’.

 

The Social Services and Well-being Act (Wales) 2014 aims to give those who receive care and support a greater voice and more control in their care. This means that you are an equal partner in the care you receive, and we will work with you to move towards the outcomes you want to achieve. We will talk about the things you are important to you, and have a ‘What Matters’ conversation with you. We will also work with your family and the people who know you best.

 

A ‘What Matters’ conversation will involves finding out a bit more about 5 key things:

  1. your circumstances
  2. your personal outcomes or what you would like to achieve or change
  3. anything that is getting in the way of you achieving those outcomes
  4. what the risks are to you or others if those outcomes are not achieved
  5. your strengths and capabilities

 

We are working closely with those who provide Domiciliary Support, Care Homes and Nursing Homes to make sure that they are putting you at the heart of the work they do. In fact, it has become a condition of the contracts we have with all of our providers, and the Care and Social Services Inspectorate Wales (CSSIW) are now inspecting against this under a new process.

 

After all, this is your life, and you should be able enjoy the things that are important to you.
